Below is the letter sent to the patients whose addresses I had access to when I left Davis Street.

If you have any type of Medi-Cal Plan, you can see me at Eastmont as well.

Unfortunately, they do not have a contract with any PPO plans. I am really saddened that this will exclude some dear patients. There are some really good Pediatricians at Castro Valley Pediatrics in Hayward and at Palo Alto Medical Group in Dublin. I sometimes work shifts in the Pediatric Urgent Care Clinic at that Dublin office.

Dear patients and families

Once again, I have to write to you about a move to a new place of work. I will be moving to Eastmont Wellness Center in Oakland on September 1 of this year. Anyone who would like to make the trek up to Oakland to continue seeing me will be able to do so. If this is too difficult for some families, I understand. Options include staying at Davis Street or finding a new medical office near your home. The Alliance should have a list if you need it.

The Davis Street Clinic was unable to afford keeping me there full time, and had cut my hours to twice a week. At Eastmont, I will be available Monday through Friday.

Eastmont Wellness is located at 6955 Foothill #200 in the old Eastmont Shopping Center. The phone number for appointments is 510-437-8500. If you head north on the 580, take the Keller Ave exit, then take Mountain Blvd to Edwards Ave, then turn left and down the hill to 73rd and Foothill.

Everyone receiving this letter is listed on my current Alameda Alliance list. In order to get reassigned to Eastmont, you must contact the Alliance in August and ask to be assigned there. I have informed the Alliance that I will be moving there, but there might be some confusion initially.

If you need to get medical records, send the request to Davis Street. If you need older records that would be in my old charts prior to July, 2018, send the request to my PO Box with a check for $30. It does cost money to retrieve those old records.

I enjoy taking care of your children and watching them grow up. Hopefully we can continue that relationship over the next many years.
