To my patients (and families),

I am writing to let you know that after 31 years in private practice, I will be taking on a new challenge and closing the office in Castro Valley. I will be working in San Leandro, building a pediatric practice for the Davis Street Primary Care Clinic, beginning on July 1 of this year. The reasons for the move are many, and I and many of you will have some disappointment. At the same time, I am looking forward to the new job.

Many families will be able to follow me to the new location if desired. Attached to this letter will be a page detailing the options, depending on each family’s insurance situation. For those who are able to continue with me, I would be honored to continue as your child’s physician and the chance to watch them grow into adults. I am heart broken that I will not be able to continue as the doctor for every child in the practice.

Judy and Brianna will also be coming to Davis Street Clinic, and I know they are looking forward to seeing you there. The new office address is 3081 Teagarden Street, San Leandro, CA 94577, about 3 blocks south of the Marina Outlet Shopping Center. The phone number is 510-347-4620. The website is davisstreet.org.

I will be working from 8:30 AM until 7 PM at the new office, Monday through Thursday, and I should be able to accommodate everyone who needs an appointment on those days. On Fridays, there will be several Nurse Practitioners available to see your children, particularly if they are sick.

Every patient will need a signed record release request, even if following me to the new office. We will have release forms available in the office throughout the month of June. Although records can be obtained at any time in the future, signing the release in June will make it much more efficient for you.
